WHAT TO EXPECT

A clear way forward, with your nurse beside you.

01

02

Your Initial Registered Nurse Assessment

A Registered Nurse meets with you to review your concerns, medical history, medications, goals & what is important to you.

03

Receive Your Personalized Care Plan

Your Registered Nurse helps you see the full picture, including issues you may not have noticed, and turns it into a plan of care with clear next steps.

THEN YOU CAN DECIDE.

• Most families prefer to have their dedicated Registered Nurse stay involved.

• Some families take their personalized plan of care and run with it.

• Either way, the choice is YOURS and you can change your mind anytime.

WHY?

Because what happens between appointments can change everything.

The Reality

  • Nearly 1 in 3 older adults sees five or more doctors each year—yet no one may be connecting the whole picture.*
  • Older adults take an average of 4-5 prescription medications daily, with many taking 10 or more.*
  • 1 in 4 older adults fall each year.*
  • More than 1 in 5 hospital discharges result in a preventable readmission within 30 days.*
  • Many adult children live an hour or more away from aging parents.*
  • 77% of adults want to age in place at home but don't know how.*
  • 1 in 3 older adults in the U.S. live alone.*
  • More than 1 in 5 Americans are unpaid, untrained family caregivers.*
  • About 80% of medical bills contain at least one error.*

The Missing Guide

  • Providers treat in the office and hospital. Insurance bills. But few people see the whole picture, or walk the path with you in between.
  • Symptoms change, questions pile up & you may be left unsure of what to do next.
  • Discharge instructions can be confusing, medications may go unreconciled, & follow-ups can be missed; not from neglect, but because the system is complex and overwhelming.
  • Healthcare is built around individual episodes of care, not your specific healthcare journey. That is the gap your dedicated Registered Nurse Advocate fills; anticipating needs, connecting care, and guiding what comes next.

It's Personal

  • Every healthcare journey is different and personal — no two diagnoses, families, or paths forward look the same.
  • Goals, values, and family dynamics shape the right plan just as much as the diagnosis does.
  • What works for one family caring for a parent won't work for another navigating a new diagnosis alone, and culture, finances, and geography all shape what the right care looks like.
  • Many families know they need help but don't know where to start, how to get it, or what to ask.
  • Our Registered Nurses bring both professional expertise and personal experience. We understand not only how healthcare works, but what it feels like to make difficult decisions for someone you love.
WHERE & WHEN

Wherever your healthcare journey takes you, a dedicated nurse helps guide the way.

Where

Once support is established, your dedicated Registered Nurse supports you in your home, by video, by phone, or alongside you at medical appointments and in the hospital. We can support as little or as much as is needed, & wherever important care conversations and decisions happen.

When

Standard availability is 8:30 a.m.–5:00 p.m. Established clients may contact their nurse after hours for urgent concerns. For a medical emergency, always call 911.

Improving your healthcare journey, one step at a time.
